Our community is unique. Our approach to improving community health is unique, too.
What is our approach?
- Every three years we conduct the Community Health Needs Assessment in partnership with our non-profit hospitals.
- We build data-driven action plans to guide our community work.
- Our team works alongside local partners. Our shared goals move us toward a healthier community.

Current initiatives
Community Transformation
Community leaders developing solutions that lead to connection and a sense of community in Clifton.
Spanish Advisory Group
Hispanic leaders guiding organizations on how to serve and build trust with our Hispanic and Spanish-speaking residents.
Substance Abuse Prevention
Community coalition working to prevent substance misuse, with a focus on youth substance use prevention.
Suicide Prevention
Promotes and implements community-based strategies for suicide prevention including access to care, lethal means safety, increased connectedness, economic stability, education, and postvention services.
Trails
An outdoor recreation partnership supporting construction, planning, and maintenance of non-motorized trails in Mesa County.

The Clifton Community Commons Vision Plan will serve as a long-term action plan to connecting and enhancing existing schoolyards and neighborhood parks, improve access to the Colorado Riverfront, develop safe routes, and expand natural open spaces.
